
Lakeland Volunteers in Medicine (LVIM) is proud to announce that it was
recently awarded a $25,000 grant as part of a $100,000 Health Initiative
offered by the United Way of Central Florida. The grant will be used for
the Chronic Disease Management Clinic within Lakeland Volunteers in
Medicine. Some of the chronic conditions treated are hypertension,
diabetes, obesity and hypercholesterolemia (high cholesterol). Having
one clinician manage all aspects of the patients’ conditions provides a
continuum of care which leads to measurable improvements in the health
of their patients.
Angie Crozier, ARNP and director of the program, says, “This money will go a very long way in treating many of LVIM’s patients who experience not only one but two or more chronic conditions rendering them very vulnerable individuals.”
